Feels like something changed recently. Opus used to handle in-session context pretty well, but now it drops obvious links. Example: I say “Edit A & B” → it forgets A belongs to Table A and B to Table B… even though it just worked with both in the same session. It’s not a long context issue — it’s basic continuity.
