Do you have this too? From what I have figured: Start with planning mode -> everything fine, no edits. Then accept and implement with the new smart “accept all” feature. Afterwards I have a larger follow up change and go back into planning mode. Now Claude Code wrongly edits as much as it likes rather than outputting a plan. When I complained about it, 2 or 3 prompts later it said “oh we are in planning mode, I will not edit”. submitted by /u/rabandi
