We had the classic 2026 nightmare: AI coding agent given a migration task, context drifted, and it issued a full production database delete with no confirmation step. One API call later — everything gone. Recovery took days. I gave the pattern one thumbs-down in ThumbGate. Version 1.19.0 auto-promoted it (with new TTL so rules don’t accumulate forever) and the next attempt was blocked at the PreToolUse hook before any tool executed.
