There is something wrong with Opus 4.8. It cannot follow instructions well. Tool calling fails and I’m wasting a lot of time stopping its current session and telling it to re-read the SKILLS.md file. Anyone experiencing the same issue? npm: v2.1.158 submitted by /u/redditslutt666
