I don’t know what it is, a model problem or a harness problem like others have suggested, but i’ve had a very bad time using Opus 4.8. I’ve got both Codex and Claude x20 plans and I’ve got a workflow where I use Codex as main planner and executor and it asks Claude for multiple round of reviews (some in parallel with different review topics) for both planing and executing, until they agree on the outcome. Also I use Claude pretty much alone for UI polishing. With this workflow I max out both subscriptions weekly and it works pretty well. But since I upgraded to Opus 4.8 I had multiple problems I never ran into before: getting stuck failing to read files, failing to output both to files or in TUI, failing to edit code, failing to ask questions (even though it wants to) and even derailing on reviews so much that Codex choose to just ignore it a couple of times. This never happened with Opus 4.6 or 4.7, but multiple times in just 3 days with 4.8. Though I haven’t tried yet to go back to 4.7 to see if it does the same now.
