I took the time to plan out things in plan mode with Claude code (Opus 4.8) and in it we defined the steps to execute the various parts and considerations. In it I had the phased build, sub-agent plan, testing and QA. I then had is save the file as the typical plan.md I then started a fresh session to have fully cleared context, prompted it in a order to fully read the claude.md , then plan.md , then requirements document, database schema plan, and UI design file. I then gave it explicit instructions to follow the plan.md prescriptions and to read it in full, don’t guess. Then told it to start work. Its still constantly asking what it should do next, I remind it to read plan.md and it keeps saying “I haven’t been, let me check it now. Yes I see this is already planned, I’ll follow it” I then will see it doing stuff or not doing things that I know are counter to the plan. Things like, are you using sub-agents like we planned in plan.md? and it said back "Fair question, and an honest answer: mostly not yet, and that’s a gap I should close now. " I’ve also asked it how to resolve and it says I’m already doing what I can and then says it will do better. I’ve broken the phases up and save status and decisions in the typical ways, have it close out loose ends, update logs and notes, push git so it doesn’t run into running into running out of context and losing things in compaction. I’ve tried documenting everything, then triggering a manual compaction. I’ve also closed out sessions to start fresh and it seems to start strong. Kinda at a loss, why plan when it won’t follow it and also I really need it to follow these plans because it keeps us on track. Any advice? submitted by /u/ryan112ryan
