If you haven’t seen it, Prompting Claude Opus 5 - Claude Platform Docs Anthropic released a guide on improving instructions for Opus 5. The TLDR: a lot of stuff that used to be required, isn’t any more. Go through your instructions, skills, etc. and cut it way down to just the objectives and key requirements.
