Seeing a bunch of variations of this: Decision.md one ADR file per architecture choice, a remember.md for context handoff, roadmap/backlog files, some updated manually, some auto updated by some kind of setup. For anyone who runs this for more than a few weeks, does it stay useful as the project grows, or does it get unwieldy? Too many file stuff going stale, the AI reading the wrong one , the wrong time?
